I vaguely remember David Simmons having a stupidly good year circa 2013. Ben Barba probably deserves an inclusion somewhere.
I've probably been a bit liberal with 'wild run of form' and putting dayne Weston down, but there was a season there that he was running for 100 odd metres, 10-11m per run with 30 odd tackles off the bench. I remember Scott Bolton being a weapon second rower one year too, Gavin cooper esque glbut arguably better. Then they seemed to pack 10kg on him and make him a front rower. Joe Galuvao probably another. Untouchable in 03-04, went to south's where he could barely get a start, I'm pretty sure think they tried to slim him down for more minutes.
All sorts of weirdness in 2001. Preston Campbell as Dally M winner. Mark Hughes as NSW fullback after playing there a handful of times. Jamal Idris came to mind as well, I remember one season he was battling with the likes of Inglis and holding is own and then only one or two later he was awful.
02-03 Richard Villasanti, went from being a bit part player at struggling teams to a grand finalist in 02 and had a monster year in 03 making the Australian side and stole all his teammates cash on tour.
